 Original Computerized Passport Valid upto October 2012

1. Copy of CNIC with Original English Translation
2. Any one local Utility Bill of December 2011 or January 2012
3. Proper Indian Address with phone numbers (only for Surat & Mumbai) with Utility Bill of Dec 2011 or Jan 2012 as per address given
4. Previous Passport required if Indian visa is not stamped on current  Passport
            5. Unutilized Visa form if any.
